Preparing for Your First Visit

hey I gladly accept insurance and bill insurance companies directly as well as provide care on an out-of-network and cash pay basis.
I am in-network with many employer sponsored plans from these insurers, but few of the individual market/ACA plans for 2018: - Premera/Blue Cross/Lifewise - Regence/Blue Shield/Bridgespan - First Choice Health Network/Moda (includes a few Kaiser Permanente PPO plans) *Please check with your insurer before our first visit to ensure I am in network with your specific plan. ** For other insurance carriers not listed above, I am considered “out-of-network.” This means that clients will have to pay for services “out-of-pocket”. I can submit the claims to your insurance in most cases for any out-of-network reimbursement available with your particular insurance plan. |
During our initial visit, I will start to get to know you and try to understand what you are seeking to make sure we are a good fit for working together. I will ask questions about your history including physical and mental health, past treatment, relationships, etc and listen to you. We may need more than one session to complete the initial assessment. At the end of the initial appointment, we will begin to collaboratively create a treatment plan for our work together. If you need to cancel the appointment, please provide at least 24 hours notice. I may not be able to reschedule our intake appointment if less than 24 hours advanced notice is given for the missed appointment.
I will have some forms waiting for you 15 minutes before your first appointment on a clipboard in the lobby. However, I will also be sending you a link for gaining access to a Patient Portal from my electronic health record system, Valant. Please click on the link within 72 hours and enter the 4 digit code I will give you. There, you will find two additional forms I need you to complete before your initial visit. Completion of those two documents is very important for making sure we can make the most of our time together. Please call if you are having difficulty accessing the Portal or need me to reset your password. If this is causing stress, let me know and I can email you the forms or have them waiting for you.
